This is the one that I used and liked. I hardly checked anything, but I just liked the format of it for the few things I did want to relay to the medical staff.
Not using one as every friend and family member I know had theirs go by the way side when sh!t hit the fan....so many of them were upset that they didn't get to follow everything they wanted due to unexpected circumstances. ?I'm just going to go with the flow, screaming "GIVE ME DRUGS!"?

My hospital just has a Birthing Preferences form I used last time and will use again this time. It asks 3 basic questions:
- Personal information you would like your labor nurse to know about you (example: past life experiences that may effect this birth; fears; feelings about pain medications):
- During labor & birth, I would like (example: pain relief options, role of birth partner):
- After the birth, I would like:
My answers are simply about how quickly my labor progressed last time and it makes me a little nervous for the 2nd time (since I heard they go even quicker). And that I have really bad veins so starting an IV is difficult.
I would like to be offered pain meds options prior to epi but want an epi again. DH is always to be in the room with me.
After birth, I would like to BF asap and DH would like to cut the cord.
