RP: NE1 have Kaiser HMO with IVF coverage?

There is a rumor that my company will have this for next year.  Were there any stipulations for approval of your IVF cycle?  I am worried with my 2 failed cycles and 2 cancelled cycles we might not be approved.

Our HMO coverage through Kaiser included nothing IVF related, and in most states, they provide zero IVF coverage.  They don't even have the facilities here.

We have Kaiser HMO and IVF is covered 80 %.   I know there were some stipulations but I 'm not sure what they were.  Our Dr. submitted the request.  We were referred to an RE outside of Kaiser.

I am an employee of Kaiser so they paid 80% of our IVF cycle...but you have to be an employee for fertility coverage.  That would be great if they covered for regular memebers as well!
