In the beginning, the measurements between A and B were always off. Usually by anywhere from 3-6 days. (I've had ultrasounds every other week since 6w3d, so lots of them.) It wasn't until I went for my 19w that they both showed exactly the same. It also depends on who is reading and measuring and the kind of machine (meaning that obviously the radiologists in the radiology dept can give a more precise reading than the u/s machines that sit in each room. At least this is what I have been told.)
Very normal. They do have a range - and the ultrasound tech will probably tell you what the range is if you ask. Mine told me that it's something like 17 days difference before they are very concerned, and they watch closely when it's more than 10 days difference. They take into account the variances in measuring that are just normal when you're measuring two moving objects!

Totally normal and common. Early on, mine were measuring 3-4 days apart and then they got closer in growth throughout the pregnancy. At my last ultrasound (32 weeks) they were within a few ounces of each other (estimated). But even if they were further apart, they don't get concerned unless they're REALLY far apart. Every baby is different and will grow at a different rate. Also, ultrasounds aren't perfect.
Re: measurements off?
My MFM and OB always warned me that a discrepancy between twins in the early months can be completely normal. I wouldn't worry if your doc isn't.
