My diaper bag was stolen and my daughters shot card was in it. How to I get a new one? We go to naval on pendleton if that makes any difference. TIA

    I don't know for sure but I would guess you could get a new one from medical records. They should have all the info to fill out a new card.

    I'd ask the clinic you go to.
