Bad Rash

We just started using cloth diapers in the last week. DD has not had a rash in about 6 months and this morning she has a rash so bad the skin is broken in one of the spots =( Am I doing something wrong? What should I put on her? How do I strip the diapers...we have a HE top loader so I know it's different then regular methods.

What diaper did she have on this morning? 

Are your diapers new? or used?

They are a mixture of New/Used.

I do a hot wash cycle with just vinegar, then a regular wash with RNG detergent and vinegar and an extra rinse.

Did the diaper this morning smell badly of ammonia?  I've had an open sore pop open overnight from an ammonia burn.

Some babies can't handle synthetic fibers.  What kind of diapers are you using?
