I'd say around 3. Things like amblyopia (lazy eye) need to be corrected by age 7 or else not much can be done. I feel bad when older kids come into my office with this and I know the Dr can't do much!

I would say age 3 also, if you dont notice any problems. My now 3 year old DD had surgery on both eyes last october to correct exotropia (eyes turning outward). I'm glad I got it done early.

I took my DD at 1 1/2. Insurance fully covered it then so I figured I should just do it. I'm glad I did for peace of mind. They did minimal testing so it didn't bother my LO. They get stickers and books and point and have them look at things while the doc holds an instrument that can measure things in their eyes. I won't go back until she's 3 or slightly later.
I can't remember if it's 3 or by kindergarten. DS has been 3 times already, but we noticed one of his eyes turns inward. So far we are just monitoring it. Of the 3 appointments it was only the last one this summer that they were able to get him to really participate and determine his vision level and that his eyes are equal. I give pediatric opthamologists a lot of credit. DS cooperated with a few tests when he was younger but was over it before they could finish. It was enough to know it wasn't a serious issue though.
