Questions to ask RE

Hello ladies.  I'm meeting with my RE again today to go over some of the questions I have and to make a plan for our first IVF.  I know this gets asked a lot, but I'm trying to make sure I ask all the right questions.  I copied the ones from the links Mayan shared, but didn't Edwina have a great list too?  Does anyone have the link that to that post?  Or others that have the list?  Thanks!  Sorry I didn't save them when they were posted the first time.
