Different IVF protocols?

My beta is on Monday, and I must say, I'm not exactly feeling hopeful. So I'm trying to look ahead to what our last cycle could be like. I don't know the names of the protocols I've used so far, but I can tell you a little about them.

IVF #1 - Stimmed with Follistim and Menopur for 8 days, added Ganirelex on day 5 or 6, triggered with Novarel on day 9.

IVF #2 - 4/11-4/22 10u Lupron, 4/23-5/4 5u Lupron, began stimming with Follistim and Menopur 4/23-5/4, triggered with Novarel.

What would you call these cycles? What others are there? I'm just trying to get an idea of how stressful my summer will be. thanks.

We also glory in our sufferings, because we know that suffering produces perseverance; perseverance, character; and character, hope. And HOPE does not dissappoint.

I know this is antagonist protocol- it's what I've done the last 2 cycles.

I pray you don't need to worry about a near future cycle. Best wishes at your beta! xoxo

  • Your first cycle was antagonist.

Not sure about the second, I've never used lupron. Not sure if it would be long lupron or micro-dose lupron.

  • I've done both of these protocols.

First is antagnoist and second is long lupron.  Those are the only two I've done so I can't help with any others.

I really hope you don't need to worry about this though!  Good luck!

  • The second one is long lupron. The microdose lupron does 5u am and pm on lupron.

Madelyn or Matt'sbride - do you know what our clinic calls the atagonist protocol? The nurse I talked to gave me the letter or number comob of the one I will be doing and when I asked her what it was she said she didn't know.

After 3 years of infertility we were blessed with twin girls through private infant adoption.

  • 1st cycle sounds like antagonist -- that's what I'm doing now, well I'm doing a variant of that protocol.

2nd cycle sounds like long lupron -- that's almost exactly what I did for IVF#1.

The others I've heard of, that I don't know much about are estrogen priming, agonist, and flare.

I really hope this cycle surprises you and you don't have to worry about IVF#3.  Good luck!

Dr. H called it the antagonist protocol when she first told me what I'd be doing. And then, when we had our WTF, she told me if we cycled again we would do micro-dose Lupron.

It really seems like our clinic is totally different with me than with y'all. LOL!

I think I remember seeing "S-Protocol" at the top of one of my calendars. Does that sound right? It bugs me that they don't call it the same as other clinics...so confusing! Sorry I'm not more helpful, but thanks for your help!

  • agonist = lupron, usu. refs long lupron

    antagonist = ganirelix or cetrotide

both are named for how each drug prevent's you from ovulating.

Estrogen priming, is just priming and can be done many many ways, but Cornell does the LP Ganirelix version (aka patch protocol), and it's ADDED to a stim cycle. So you can do EP with antagonist, or SIRM does an agonist-antagonist conversion, or you can do it with MDLF, etc. Aggressive.

MDLF Microdose Lupron Flare, takes advantage of the flare effect of FSH production that is a byproduct of taking Lupron. Aggressive.

Also Clomid+Injects protocols, like what Patchen did.

ez or miniIVF for low dose meds, some people don't do well with high stims.

There are so many protocols, and variations on how to execute these protocols, what drugs what doses, constant vs step-down, etc.

Well this wasn't Janet I was talking to. It was a nurse that had called me back after I had left a message to see if anything changed. She was just looking at my calendar and told me what it said.

Humm, the one she said on the phone had a number w/ it so I guess I'm not doing that one. At my WTF back in Feb. they gave me a sample calendar for AL2 protocol which was the microdose lupron flare. That isn't what she said either. I guess I will have to wait until my phone consult on Monday to find out what I will be doing.

Your IVF#1 was a standard antagonist protocol.

Your IVF #2 was a long lupron protocol.

These are the two most common.  There are variations to both of these including a low dose antagonist protocol where you would start half doses of the antagonist (such as Ganirelix) before or simultaneously with starting stims.  They can also do those with EPP.

There is also a micro dose lupron flare protocol and a agonist/antagonist conversion protocol (with and without EPP).  I'm sure there are others, but these are the ones I have discussed with my RE and my second opinion RE.

I'm still sending lots of T&Ps and PPVs your way that you get good news on Monday!  Good luck!

  • Ditto that you did Antagonist/ Long Lupron. There is also:

Estrogen Priming is usually done with an antagonist protocol and with estrogen patches beforehand.

Flare/ micro dose Lupron flare/ Lupron flare is closer to Long Lupron with a micro dose of Lupron instead of the full strength Lupron. This can also be done with estrogen priming.
