Yeah, they give you an HTML code that you copy & paste into your blog's format area. It depends on what blog site you use but there are instructions on how to do it.

I pay someone to do my blog design. I use Penny Lane, Marina is wonderful to work with.
But, you can go to sites like cutest blog on the block, shabby chic blog designs, etc and get free backgrounds!

If you use blogger go to your design page
click on add gadget and scroll and clock HTML/JavaScript
when the screen pops up paste the code that you get into the large box.
