Remember, you don't want to massage up near the urethra, just the bottom half down near the rectum. Otherwise you could tear into your urethra which is a complicated fix, and extremely painful...
You don't necessarily ever have to. I remembering reading about at least one study showed no difference in frequency or degree in tearing between women who did it and those who did not.
