Warm or cold? Cold. I have never warmed up any of DS bottles. I usually make the bottle and let it sit while we do bath, and books so its not right out of the fridge. But I have given it to him right from the fridge, he is not picky though.
I eased into it. Milk bottles at home, formula bottles at daycare for a few weeks while I watched to see if she was going to be okay with that. She had a milk protein insensitivity.
We eased into it. Both my guys nursed and never really took a bottle so it went in a sippy, always cold. We started giving little bits at mealtime and them started replacing nursing sessions with a cup of milk.

We kept going with milk in the bottle until about a month ago. I only warmed the milk at night for the bottle. I still slightly warm it in the sippy cup now. Now we're all sippys, all the time, night time sippy is warm milk.
And also, in case you're wondering, Laurel would only use straw sippy cups for the first few months. Only around 13 months did she start to use regular ones.
We tried it every once in a while after she turned one, but E hated cows milk. She loved goats milk, so she drank that for a while. She could drink out of a straw so we just helped her with the glass. She never got to just carry it around (I was paranoid about it spoiling or finding it in weird places). Then at about 18/19 months she started liking cows milk. We have a Klean Kanteen with a sippy adapter and that's been great for keeping the milk colder, longer and not giving it any funky smells. We never heated it.
