I'm guessing you're going to tell me that if I don't want to futz with folding a PF, I should just buy a fitted. As a cheapskate, I'm wondering if I can't just use the eleventy billion microfiber inserts I already have instead of a prefold...

I have seem BG sleeves for MF that would allow you to just use them in a cover. You could also lay something on top (fleece) but I would be worried that it would shift and expose the baby's skin to MF.
You can, however, tri-fold a prefold and lay it in the diaper cover like you were thinking of doing with an insert... I haz no baby yet, but I know this is often done. See pics in link:
