Usually a giftcard to where they are registered.  That way they can use it with the 20% of completion code.

I like to buy the baby bathtub and some bath must haves. Temperature duck, towels, and baby soap.

Ha! Sleep deprivation is horrible I thought you were asking about a baby shower!

So, for bridal showers, I like to buy kitchen stuff. Pots, plates that sort of thing.

This. I buy them something they asked for.

I give them something off their registry but packaged cutely.  My two go-tos:

1. The towels folded nicelt into a cute basket or trunk (depending on your price range). I've used the laundry basket if they register for one. I usually add bath stuff.

 2. Baking pans and spatula with a cute apron from etsy and/or Julia Child's Joy of Cooking (or another staple cookbook).

I also love to give their towel sets. It's a big gift (yeah I'm shallow like that) and it's so pretty to open with the colors (assuming they chose colors!) Second choice is usually a gift basket filled with their miscellaneous kitchen supplies, like ice cream scooper, measuring cups and spoons, spatulas, etc.
