I noticed DD squishing her blanket up under her head a few months ago and bought a small pillow form from Walmart/JoAnn's. DH sewed a pillowcase for it and we gave it to her. She must hate it b/c she throws it out of her crib. We haven't given it back to her in months after bunches of failed attempts. As long as the pillow isn't big enough that they're using it as a step to crawl out of the crib then I think it's safe.
I don't know when it is considered safe, but we started at about 15 months. Only because DS would bundle up his blanket or use a stuffed animal as a pillow. I figured that an actual pillow couldn't be an less safe!
DD's been sleeping on a regular sized (slightly thinner) pillow since 13 months. She sleeps ON it about 75% of the time (the other 25% of the time she sleeps straight on the mattress).
DD had a pillow as soon as she went into the crib. At a month old. But when she napped in the crib sooner she was on pillows. They were under her sheet. Plus, for our health we both have to be tilted just a little bit.

Since about 9 months, he was sleeping in bed with us and used a pillow. We moved him to his crib about a month ago and gave him a small flat pillow and he uses it in there too.
