I open the car door for him, and I strap him in. He knows how to release the seat belt. Yes to the pencil. He doesn't pee during the night, but if he needed to go, he would wake me up.

The car door is still a bit heavy for her and she struggles at times. But she has been strapping herself in for about 6 months now.
Pencil hold - about 6 months
and bathroom in the middle of the night doesn't happen. But she does go around 10pm, before she goes to sleep (after laying in bed for 90 minutes) and she comes in our room to use our bathroom (even though there is one closer to her room).
K turned 4 in October so she's not quite 4.5 but I'll answer anyway:

He can open the car door, get into the seat, put the straps on and buckle the top buckle. We've never tried having him buckle the bottom one.
His OT is working with him on the pincher grasp. He can do it if he's reminded, but he will grab with his whole fist almost every time first.
Yes, he can get up and go to the bathroom on his own in the middle of the night, but he's very fearful of monsters lately, so he probably wouldn't.
