Trouble TTC

IVFers - help please!

Okay, tomorrow I have my initial consult with the Clinical Trial.  I just feel so lost though.  I have no idea what questions to ask or anything.  I feel like I know so much about everything else IF related but I'm totally clueless with IVF. 

Any help would be greatly appreciated.  Thanks!

    This is the list I had put together from all my research for my consult.  Many of the questions he answered before I had to ask them.

     

    What tests do we need to complete or have re-done before we can start a cycle?

    Based on my records and how I have responded to injectables do you know what kind of protocol you would be recommending I do?

    How often do you start new cycles? How long do you generally keep a patient on BCP and Lupron? (He was able to answer this for the most part because he had already reviewed my med records and is 90% sure I am will be on long lupron protocol.)

    At my age and given the fact that we do not have an genetic disorders that are known of at this point would you recommend PGD? If for any reason we did any up needing PGD what costs are associated with that?

    Do you prefer a 3dt or 5dt? How many embryos do you usually recommend putting back in each of the two scenarios?

    Do you offer a shared risk program? (I knew mine did.) Do you think I would qualify to participate in this program?

    What is criteria for triggering? Do aim for a certain amount of follicles? What circumstances would warrant canceling a cycle?

    I assume you will be doing ICSI -  Costs? Higher success rates? Increased risk of miscarriage or birth defects when ICSI is used?

    What do you think the success rate based on what we have discussed would be? Of that what chance of multiples?

    Is assisted hatching something you think I need to consider? Additional costs?

    What procedure do they use for freezing remaining embryos (do they use
    vitrification)? What are your guidelines for what embryos make it to freeze i.e. do you only freeze blastocysts?

    What is the thawed vs/ fresh success rate? Where are the frozen embryos stored? Do they charge storage fees?

    What are your thoughts on acupuncture increasing the success rates of IVF? Any places you recommend?

    May 2011 = Myomectomy and trans-abdominal cerclage (TAC)
    Sep. 2011 = Surprise BFP = C/P
    Feb. 2012 = sFET #1 = BFN
    Feb.2012 = Hail Mary IUI #4 = BFN
    April/May 2012 = FET #2 w/our last two embies = BFP (Please let this be it!)
    Beta #1 8dp5/6dt = 234 Beta #2 10dp5/6dt = 695 Beta #3 12dp5/6dt = 1796 Beta #4 17dp5/6dt = 17,888 U/S #1 May 17, 2012 = Twins
    Baby B's heart stop beating at 9 weeks 5 days
    Our little miracle baby is a boy. :)

    Baby Boy Owen and Baby Girl Avery were born too early on Feb. 13, 2011 due to a pedunculated fibroid, incompetent cervix and suspected placental abruption.
