Plan to stop every few hours, leave around naptime so the first several hours are spent sleeping, bring toys he hasn't seen before, sit in the back with him for part of the time and read books, sing songs etc.
Oh, and when you get there remember that he hasn't moved all day so don't play pass the baby (I assume you're visiting family), instead let him crawl around and explore as much as possible.
Portable DVD player. We took DS on a 20 hr car ride over the summer. The DVD player was a lifesafer. We watched Nemo and Lilo and Stitch and when DS would lose interest after 20-30 minutes I would stop or pause it, play with something else for a little bit then go back to it.
We had a 5 hour trip when the girls were about 9 months and we drove after they went to "bed" for the night. Nighttime routine and then into the car seats and it worked really well for us. So we left aound 8:30 and got to our beach house at 1:30 AM. Not an ideal way to travel but it worked for us and it was great to wake up in the morning and be there with rested babies!

Exactly this. We just did a 7-hour car ride, and halfway through we stopped for lunch and let DS crawl around afterward in a grassy area for about 20 minutes. He got a little fussy at the end, but I was ready to be done, too!
We make semi-regular trips to my hometown, which is a 6 hour drive. The main thing is to plan your timing around LO's nap schedule. And we have always tried to feed him while strapped in the car and then let him ans then let him run around when we stop. I usually sit in the back when they aren't sleeping to help entertain. And I pack a bin of toys appropriate for the car. Fingers crossed for you.
