OB appt - am I overreacting to this?

My OB's office has four delivering physicians, I purposely scheduled today's appointment with one of the doctors I haven't met yet.

My next appointment is at 30.5 weeks and today's doctor ordered nothing more than an in office check with a doppler.

I asked for a cervical length ultrasound and she said that nothing would indicate that a CL check is necessary. I responded with I'll be nearly 31 weeks pregnant with twins who are gaining approximately a pound every two weeks, how is a length check NOT necessary. And she basically flat out told me there's no reason I need to have one.

I left the appointment, got in my car and started to cry. I don't want a full ultrasound, but I feel like knowing that they are secure up there and doing well at this stage is very important.

A little background, I passed the GTT, my blood pressure is consistently low (today was 110/58) and my last (and only) three CL checks were constant at 2.8

If I've been consistent is it possible that it's not necessary and I'm overreacting? Feel free to be honest.

Take this with a grain of salt, since I seem to have the cervix from Hell, but...

I would settle for no less than a cervical check every month until 28 weeks, and every 2-3 weeks after that.  Basically at every.single.appointment.  Like I said, I'm jaded when it comes to the cervix, so take that for what it is.

Just from what I know and what I go through every appt...they check my cervix every single time...in fact tomorrow the doc says he has to do a vaginal b/c they can't see it anymore b/c one of the babies is blocking it..I get it checked when I go to my reg OB and the specialist I see in conjunction.

I don't think you are being unreasonable at all. Are you seeing an MFM? I'm having a cervical length check (vaginal-so they get a great look) every appointment/ultrasound I have with them, so every 2 weeks. This will continue until 36 weeks.

I only got cervix checks (transvag u/s) until 26 weeks.  My OB would have started them up again around 32-34wks but things went crazy so the last ones I had were in the hospital.

i had cervix checks every 4 weeks starting at 16 weeks... until 32 weeks. After 32w my Ob said it was not necessary.  The cervix checks were with an MFM/Peri - not with the OB. Are you seeing an MFM, too?

my cervix measured so long at one appt (it was in the 20's somewhere) that they did it twice b/c they couldn't believe it... but i continued to have them every 4 weeks- one good cervix check doesn't mean that the next will be.

sounds like your OB doesn't know much about twin pregnancies.

Are you seeing MFM? I was told I had cervical check monthly until 30 weeks by u/s and than from there on every other OB appt by the dr

I think that everyone's experience is going to be different because OB's tend to have different approaches.  In my experience I had a small amount of shortening early on (about 9-10 weeks), but after many u/s checks over the course of a couple of months, didn't progress.  My doctor's plan was to only measure with u/s when they needed to do an u/s (growth checks, fluid checks, etc), after about 30 weeks.  She said at that point, especially with a twin pregnancy a small amount of shortening can be expected.  But she did say that she would be doing an exam with each appointment, so an u/s check would be redundant.

ETA: I should probably mention though that I was on bed rest, so I'm not sure if that affected things...and I never made it there because I went on hospital bed rest for pre-e at 30 weeks.

I had my last CL check at 28 weeks.  I don't think they will check again for a few weeks.  I was holding steady at 2.5cm.

My OB's office is weird.  They only do one CL check at 20 weeks.  Then, on my u/s, if my cervix isn't funneling, they won't go any further.  If it looks like it is funneling, they will do a vag u/s.  Then I don't think they do CL checks at all until 33 or 34 weeks?

I will say that if I felt I needed one done and told them, they would do it to make me feel comfortable.  I can't believe yours flat out said no.

My dr. will not do manual cervial checks until 36 weeks, but I get one every 4 weeks at my MFM until 30 weeks when they will start doing FFN test.  They said that after about 29-30 weeks your cervix starts shortening anyway so checks are not a good indication of PTL.  I would think that if you felt strongly enough about that your Dr. would have checked.  I can't beleive they said no.

i got my last u/s check at 30 weeks and one manual check at 32 weeks, so yeah, i'd think you still need to be checked at this stage.

I only went to see my OB...no specialists.  I had a full u/s at every visit-every.single.one.

However, she did not check my CL often for some reason.

That's just my experience....I can't believe you're as far along as you are.  Not much longer hon and you're there.  I'm excited for you!  : )

I'm going to start a new topic on this one...

My cervix changed drastically from 19wk check to 23wk check (3.5 - 1.9), and it was funneling.  So, from then on I stayed on continued bedrest and alternated between Peri (incl. CL checks) and regular OB appts every other week.  At 28wks cervix went down to .5cm (funneling) and I was hospitalized again.

Before 19wks I'd been getting cervical checks, at my 13wk NT, and I think one other time before 19wks.  I'd had a Leep procedure, but I still started with a 3.5 cervix.

I guess I'm so glad they checked it regularly!  But, I also don't know what's normal at 28wks.  I assume the cervix starts to thin out gradually at a certain point.

Yours has been holding steady which is good.  When did you have your last check?

What I don't get is why they don't just do a stupid u/s and put your mind at ease.  It takes 2 minutes!

I get a CL check at every appt - using abdominal u/s.  I also get an internal at every appt (not fun, but it makes me feel better).  I was told this would continue until further notice.

It would drive me crazy not getting a check at every appt.  I would definitely politely, but firmly demand a CL check.

I had my cervix checked every 4 weeks thru ultrasound... so at each appt.

(when I went into PTL...my cervix was checked weekly.)

No I'm not seeing an MFM//Peri, just an OB.

janjag - my last check was today. I had one at 22w, 26w, and 28w. Every time I was 2.8cm

Oh, I get it now.  You got one today, but they don't plan on giving you another, at least not next time.  Hmmm, I don't know!  Pay attention to your body over the next few weeks, keep an eye on your BH.  The doc you see next time can always give you a CL check, esp if you push for it!  It doesn't matter what today's doc ordered.

My last CL check was at 31 weeks. I would definitely ask why they are not checking CL, it seems early to stop.

My MFM (at one of the best hospitals in a city known for its wonderful medical care) did not do transvag cervix checks on me, ever. His feeling was that a cervical check has the possibility of introducing bacteria and isn't necessary unless an transabdominal u/s shows a potential problem and a closer look seems warranted. It made me nervous, but I trusted his judgment and carried my babies until my scheduled c-section at 37 weeks.

    Good luck!

I had an internal at every appt from 28w on, but I only had my cervix measured about every 3w.  Personally, my CL meant nothing.  Mine was thinning consistently until my 32.5w appt.  At that appt, it actually lengthened, but guess what, that's the day that I went into PTL and was dilated (so for me, my CL wasn't an indication of what was to com).

I just saw a regular OB, and they checked cervix at the growth u/s every 4weeks.  They only did ab checks and said they'd do transvag if a closer look was warrented.  Fine by me.  No thanks on the dildocam!

My CL was only checked once throughout my entire pregnancy.  It was at 19w when I had my big U/S.
