transitioning from EBF to FF?

for those who have transitioned from EBF to FF, a few questions...

-when (baby's age) did you start/finish the transition?

-how did you transition?

-what formula do you use and why did you pick that one?

    -when (baby's age) did you start/finish the transition? 5 weeks 4-5 days (it was yesterday)

    -how did you transition? We made a bottle and gave it to LO (on dr's orders) I continue to pump.

    -what formula do you use and why did you pick that one? Similac Sensitive. LO is sensitive to dairy in my diet so we thought it might be a safe choice.
