WTF set for next Wed! & a Q

Surprisingly, Dh and I only talked about IF for a whole 10 minutes during our many, many, many hours of driving over the past 5 days.
In a nutshell, he wants to talk to our RE about moving onto IVF, so I called the office today and made an appointment for next Wed. afternoon.
Now that I have this appt. set, I need to get prepared. Can you ladies give me any suggestions on what to ask the RE, as I am a newbie to IVF?

I have no experience with IVF, but I wanted to wish you luck at your appointment. I hope you are able to get answers and formulate a clear plan.

What protocol does he recommend and why?
How does he think you will respond given how you did on your IUI?
What are your odds of success with IVF?
Do they do ICSI? Does it cost extra?
Do they do 3dt or 5dt and why?
Do they cycle patients together?
Will you have to do BCPs first and why?
Just some questions off the top of my head.
