Why does anyone need to know? The nurses and dr know, and any visitor knows you. And as far as taking it home, no way. Too many stories of abductions and attempts. It's like advertising, hey, there's a little baby and tired people living here! Come n get it! No announcements, storks, balloons, nothing.
I know this is an old post but I wanted to say that at my hospital, the only time I've seen something hung on the door was to indicate that the baby was born sleeping or that the mother was grieving.
I've seen them on hospital doors. All I have seen are cute and not huge. Our hospital has a monitor bracelet system so babies are safe and actually rarely leave the mother's room. I say to each his own. We are celebrating a life! I won't make one but if I happen to get one I will use it.
Having a wreath on your hospital door is very common where I am from as well, and I live in NC. I use to work in Labor and Delivery, and I would hardly ever pass a door without one. From what I noticed, family members or friends were the ones who usually brought the wreaths in. I agree, it is probably going to be the last thing on your mind when the time comes, but hey if your a crafty individual and have some time on your hands,go for it. I think they are actually cute!

In my area, you definitely don't make them yourself. A friend makes it for you. Tacky to make it yourself.
