When the pocket diaper is wet you change the whole thing.  You only need one insert per pocket diaper since the whole thing gets changed.  Kellys Closet is great.  Generally the pockets come with inserts.

The only pocket diapers that I have received without an insert included are the Happy Heinys sized pockets.  I got an xsmall and a small and neither came with an insert.  Most pocket diapers come with microfiber inserts.  Some allow you to upgrade to hemp inserts (Rumparooz, Knickernappies).  I do like Kellyscloset and have placed multiple orders through the site, although shipping from KC is not as quick or cheap as alot of other sites.
