I'm filling out the hospital pre-registration forms and it is asking about advanced directives. Have any of created or considered a formal power of attorney or a living will?
I'm have power of attorney for both of my parents (both have had recent health issues that required extended hospital stays), but the hospital basically had forms that they filled out and had notarized.
I know that I could google it, but any personal experiences putting the documentation together would be appreciated. I know that it is hard to think about during this happy and exciting time, but I can help thinking that it is the responsible thing to do.
