2 week notice for day care provider

I'm writing up my 2 week notice for our day care provider.  She works out of her home and requires the notice for leaving to be in writing.  Has anyone typed one up and how formal were you?  I was thinking something like, this is our two week notice.  DS's last day with you will be Sept...2010.

Anything else I should include?  Thanks.

    Were you happy with the provider? If so, I'd say something slightly more personal, like you'd be happy to be a reference for her in the future.  If not, keep it short and sweet.
    Wife, Musician, Fed, WW-er, and Mom of three little kids - not necessarily in that order.

    Essentially, yes that is what we wrote. We added a paragraph with prorated tuition however and a pleasant note on how we wish continued success for the daycare.
