I think many times they just have non-PG models wearing fake bellies that they can edit to look (vaguely) real.
I know... I have a few friends back home who are as tiny as Twiggy, and while their bodies stayed pretty small with a beach ball for a belly, they did all gain weight in their faces and necks. I guess it affects us all differently, but as far as I'm concerned only very few women stay anything close to that size!
Uhg, I hate those kinds of pictures, I doubt half the women that are fully clothed are pregnant at all. Even DH asked me one day while I was browsing in Motherhood, "Do you think those models are actually pregnant?"
I will play devils advocate on the heels though. I still wear heels to work every day and to weddings and such with no problems. I think if you're used to it and you're not trying to break in a new pair of heels you'll be ok until the belly's really big or your feet are really swollen.

My ass has gotten so big! I used to sit on DHs lap and he would complain that my butt bones would hurt him.,.now he's like "you are all comfortable"..thanks DH.
I would kill to wear heels right now! My feet are so swollen that even my ballet flats won't fit. It's a good thing it's summer and that I'm a SAHM!
