Intro Post - 1st RE appt - long

Hi Ladies, I usually post on TTGP and I posted my question on TTGP but I thought you ladies might be able to help me. I had my first RE appt yesterday after I have been trying for a year and had 1 chemical pregnancy in the very beginning of TTC. I felt so knowledgeable going in from everyone here. After going through my charts and my history and my DH's SA which he had done already, the Dr. said that he thinks that I am not ovulating or not developing mature follicle and this is why I have not gotten pregnant yet.

His course of action would be to call on CD1, have CD3 bloodwork and a vaginal ultrasound. If all is ok he will start me on Clomid. Of course I asked if I would be monitored on Clomid and I definitely would.

I am happy with this course of action except that he mentioned that he would not perform a HSG test because I did get pregnant before he does not think my tubes are blocked. He said he would perform the HSG test if I did not get pregnant within 3 cycles of clomid. Actually I am going for a 2nd opinion in August b/c I am not sure which practice I want to go to.

I am hoping I will get my BFP this cylce or next before I begin treatment, but if not I need your opinion if you thought it was ok he didn't want to do a HSG initially.

  • I'm glad your appointment went well! I hope you get some answers with your CD3 bloodwork. 

    My RE required a HSG between CD5-12 of my first clomid cycle (as in, she didn't put me off a month to do the HSG).  I do think it makes sense to have one done, you never know what can happen in your body. And it's been around a year since your c/p.  But to me I see the risk of it being low and if you like your doctor, I'd probably just do it after 3 cycles as he suggested.  I think I'm probably in the minority on that, though.

    I personally would want the HSG, because one BFP doesn't mean both tubes are clear and I'd want to know my chances as best as possible.

  • Welcome! So glad your first appt went well...they can be overwhelming!

    My OB performed my HSG just before my first RE appt. Though I haven't had a BFP at all, my RE requires an HSG before he will begin any treatment. I strongly believe that we have to be our own biggest advocate...if you feel like you should have an HSG done prior to any treatment, then I would insist on it (and consider that when deciding on your RE.)   • I had an HSG first - along with DH's SA, then CD3 bw and u/s.

    I'd definitely request the HSG before going on Clomid - or during your first month of Clomid...

  • That's good that you're seeing an RE, will have CD3 b/w and u/s, and your H has had a SA already.  I don't know if I completely agree with your RE not ordering an HSG before starting clomid.  Most insurance companies put a cap on how many clomid cycles in a lifetime, i.e. 6 cycles.  So, if you would have something blocking your tubes, it doesn't make sense to wait 3 clomid cycles before having an HSG done.  Issues can pop up in our bodies at any time.  Just because you got pregnant once before, doesn't mean there isn't something wrong now.  I got pregnant easily 2.5 years ago, but then had trouble getting pregnant after I lost that one.  I kept thinking, there can't be anything wrong, I got pregnant easily before.  But, it turned out that I had endometriosis and uterine polyps.  Our bodies and cycles can change at any time, so it's better to be safe than sorry, so I would recommend pushing to have the HSG done before you go on any medication.

    Thanks everyone! Your posts are making me feel better. I am definitely going to ask this question when I go for the 2nd opinion. Based on that I think I will probably decide to have the HSG done before starting treatment.

  • Welcome! Your doc might be trying to save you money if you are OOP.  It is an expensive test.  If you have insurance coverage, I would push back and request the HSG to be done.

  • I also just went to my first RE appt last week and so far we completed CD3 bw and ultrasound, genetic bloodwork, SA, HSG and have to schedule the Endometrial Biobsy. (Appointment was on CD1 so timing was perfect). I had a chemical pregnancy in Jan of this year but RE still requested the HSG for me. So far I'm liking this doctor's approach of waiting for all test results before recommending a course of action.

    The downside is that I have to wait for all tests to be complete before I get results

  • I think I'd also want the HSG. Even though HSGs are not used to diagnose polyps, my HSG showed a small filling defect that turned out to be polyps. I had a hysteroscopy last month to remove them, and we are planning IUI this cycle. 

    Our RE had us schedule the HSG practically before our first meeting. I'm glad he did because had we just commenced with clomid we would have been wasting our time. Now that he's recommended IUI I'm even more thankful that we had the HSG and the hysteroscopy, because removing polyps can increase the success rate with IUI. The HSG was very fast and painless except for a tiny bit of cramping when they put the catheter in, and the benefits of it were big. Even if yours comes back normal you and the RE will then know what's NOT causing problems for you guys.   

  • I just had my first RE appt two weeks ago, my RE insisted on the HSG before starting me on Clomid.  I am having the HSG in Aug. and the Clomid in Sept.  She said there was no point wasting the money on Clomid, until we had the all clear on the HSG.  I ovulate on CD 20 of a 28 day cycle.
