I finally contacted my Aflac rep about what I needed to do to notify them about baby. They don't need to be notified pre-birth. And this is what she said about the claim form..

"Your physician will complete the page requiring dates of service, and all of the other pertinent info, we also need the itemized bill from the hospital. So the admission and discharge papers are good but we will need the actual bill also."

She said baby can be added at the time of birth and if LO is admitted for any reason other than delivery Aflac will pay for those hospital days too.  My friend said her cousin had Aflac on her babies' 3 month NICU stay is the only reason I thought to ask.
