Faithfulness is the dealbreaker for me.

I don't know that I would leave him (all dramatic, packing the bags in the middle of the night-type fashion), but I just can't fathom how it would work to stay together for any length of time.  

Loving someone doesn't work when they don't love you back (in that intimate of a setting).
