bookmarks for lending???

help me find:

i once saw a pad of personalized bookmarks that said, "this book was borrowed from renee143."  you tore them off the pad and it left a stub where you could write the name of the book and to whom you lent it.

anyone seen anything like this?

I bet you could find them on etsy.  I've seen book plates on there.

You can also go to Kinkos and get them made....it's not that expensive actually
