Call 1-888-222-6036.....You just have to provide the NDC number along with the lot # and expiration date and they will mail you a rebate check!

Thanks for posting! I tried to return one to walgreens and they refused. Too bad I threw it out. But I still have the ones my mom had at her house. I'm going to call those in.
