Hey ladies,
I hope each of you are doing well. I decided to come ask you ladies for some possible RE reccs for MD & for some suggestions for a friend b/c I know there are a bunch of you that know your stuff about IF & can give good suggestions. We met at an IVF support group our clinic held & have kept in touch. She currently goes to my clinic - Shady Grove, but is looking for a 2nd opinion from someone else.
Background: She has an unexplained Dx. She did several IUIs, including a couple B2B IUIs, and now two IVF cycles - all of which failed. According to what she has told me, her RE changed her protocol on IVF #2, but I'm not sure what it was (her 1st cycle was the long lupron). Both cycles she started spotting & got AF before her beta even came up - she was on Endometrin both times. Her follies, lining, egg/embryo quality all appear to be great, but she has never gotten pregnant. She thinks the problem is after the ET & getting the embryos to implant. I know that as a general rule, SG uses baby aspirin with it's patients to help with implantation + the Endometrin, but not sure if her Dr. has added anything else.
Any MD RE reccs (she lives near the Rockville area of MD) or other suggestions you ladies could give would be so helpful & I'll pass it along to her. TIA & PPV to all of you!

I would100% make the switch to PIO for post ER progesterone support - it is the only progesterone that works for me (and the only time I have stayed pregnant is the cycle I was on PIO).
She probably should ask for an endometrial biopsy to make sure she doesn't have anything going on with her lining (infection, natural killer cells, etc), and maybe the test to make sure your body produces the ?glue? needed for implantation (I can?t remember the name right now).
That is really all I can think of - hopefully she get pregnant soon!

My RE is in VA, but if she is willing to travel, I highly recommend him - he is in Reston, so not exactly close but not too bad. Virginia Center for Reproductive Medicine - Dr. Sharara. I got my BFP after I switched to him for IVF#3. I'm happy to talk to your friend about my experience there if she is interested.

Implantatin issues was a suspected issue along with poor egg and embryo quality at ET.
My RE put me on 3 days of lupron shots started 72 hours after 3dt, one shot every morning for 3 days. It is has been found to aid implantation, this was one of the changes my RE made between IVF#1 and IVF#2 along with Endometrin supps 3 times daily and 2 Vivelle patches changed every 48 hours. My RE also had me on Ganilrelix starting on day 5 of my cycle to supress me verses the long lupron I was on for IVF #1.

LCB - I was thinking of telling her to suggest the PIO to her RE. Do you have any clue why that worked better that proges supps for you? SG says that Endometrin & PIO have the same success rates, so based on that rationale, I'm sure that's why they never switched her off of that. I think it's pretty closed minded of them, if something else could work, then they should try it.
