I think someone here created a site or something where people could go to answer some questions. I don't know if this makes sense.

We are working on something to get people in my mom's old neighborhood to answer a questionnaire. There are way too many people in her old neighborhood that have passed away from cancer. Just in our immediate family there have been 7. With more diagnosed and cured. I know the health department was doing some soil samples a while back.

My aunt wants to create some type of questionnaire that she can add to a flyer for people to go to to answer the questions. We are also going to get the news stations involved so we can include anyone that may have lived in this area over the years.

Then we will be contacting the Erin Brockovich foundation. 

Can anyone help me with this? For some reason I am thing Femme, Eclaires or Goose are the one's that did this???

You can do it through Google Documents easily.  If it's multiple choice you can use surveymonkey.com

There are several sites that allow you to create an online survey.  Survey Monkey is one of them

I would contact the Erin Brockovich foundation first to see if they have a standard questionnaire that they like to use or questions that they would like you to include.  This could prevent you from having to go back to these same people multiple times for missing info.
