High-Risk Pregnancy

so many ? no answers...please help me..

ok so i went today for consult we discussed how my cl went from 4cm month ago to 2.8 2wks ago and today 2.5/6...I am 21wk 5d and I been seeing them since 13wks and she tells me today that I should have got stitch back then, I should have been taking pg shots at 16 wks but my MW or the PERI mentioned this!!!!!! so she then goes on to say that I should have had a consult to find this out but I was just referred for one 2 weeks ago!!!!! and I only seen her today because yesterday I asked about progestrone shots and the peri added  me to see the head dr today!

My cervix is like I said is 2.5-2.6 with a little funneling at the tip......

Now Im confused and need to make a decision on a cerclage (she stressed that the cerclage is only a 50%chance & things can happen like rupture/infection)or strict bedrest(with shots & still a 50% chance) &  no more work for 2 months but I have not been on my job to even be pass the probation period and my maternity leave will only be leave that I actually accumalate while working!!!!

My question to you ladies is what option did you take? and how has it worked for you? and if you got the cerclage how many weeks were you?

  • I'm sorry for the frustrations you're having with the doctor.

    My cervix started to change around 21 weeks like yours. I did not have a cerclage but I do know that other women here have had emergency cerclages at that stage. If you've had a loss before, or they know a reason you need the cerclage, they put it in in the mid teen weeks.  I did start the progesterone injections at week 16, but they knew I was going to need them because of my first early pregnancy. When my cervix started to change they made me go on bed rest, and I've been there since. I'm almost 33 weeks now.  The last cervical length I had was .7mm and I'm still pregnant, so there is hope without a cerclage. The shots and bed rest worked for me.

  • I had an emergency cerclage at 23w.  But, I was starting to dilate and my cervix was not measurable.  There are alot of risks associated with getting a cerclage later on (after 18-20w)- but those risks were outweighed for us, b/c if I didn't get the cerclage, it wouldn't have been long before I lost the baby. 

    There are lots of girls on here that have done well with just bedrest and either the progesterone shots or suppositories.  And they have had WAY shorter cervix lengths than 2.5cm.  That being said, I really feel like your perinatologist should guide you to a comfortable decision.  If they're not- I would say a second opinion is in order- and fast.

    Im sorry I left that out I had a loss of twins at 18wks....my chart had where my MW asked the head dr at ob office about shots when I was around that time and he declined saying I only had one loss....

    Oh I didn't see that in your signature either. I'm sorry for your loss! I am also pretty shocked that your doctor declined... Both doctors told me even before I got pregnant that I would have to have the shots. I don't know what he reasoning is behind that.

    I will agree with pp.  If your specialist or OB doesn't seem to be answering your questions or concerns, you could get a second opinion somewhere else.

    and thats just it everytime I asked her what would she recommend adn she kept telling me the options that she had given me not leaning towards none! she was like we cant tell you what to do because of lawsuits so we tell you your options and its up to you to decide! and I get that a little but Im NOT a dr!!! so thats why I really ahvent stop crying because Im so CONFUSED!

  • I'm sorry you're having to deal with this.  I "only" had one loss as well (with twins as well)  but my doctor was great and even though I'm only pg with one this time he wanted to be proactive and not take a wait and see approach.  He did explain that a lot of doctors aren't like that though, just in case you don't have the same issues as last time, which I think is bs. 

     I was also a lot more up front with him this time and basically took control of my care, as much as I could.  I asked him when I was going to get a preventative cerclage, like it was already a done deal.  I'll also be asking for shots/suppositories at my next appointment.  I don't know why they are so hesitant to give you direction.  I would just do my research and tell them what you want.  You're the one paying for the care.  You're the one that has to deal with it if you have another loss.  I wanted to be able to say that I did everything I could to avoid it this time.  Good luck!

    ?I can't really help you, just want to show support. ?I didn't find out I had cervix issues until I went to the hospital with contractions at 26 weeks. ?I was 1 cm dilated w/ funneling & CL of 1.7. ?I think I was past the cerclage cut off because it was never brought up. ?I've been on strict home bed rest & all my measurements have stayed the same so far. ?

    I don't know what I would do in your situation? ?You are earlier than me & had a loss so I might lean more towards it. ?I hope you find the answers you're looking for. ?

  • I am new to this emergency procedure from just a couple of days ago, but I feel like the choice for the emergency cerclage was my only option due to the fact that I was already dilated when I came in for the procedure.  With that being said, I felt informed and included throughout the process and was told of a MUCH higher sucess rate (70-80%) so long as I can make it through this first couple weeks on hospital  bedrest with no sign of infection.  I don't know what the plan will be for the remainder of my pregnancy, aside from continuing the p17 shot and bed rest of some kind.  These are not the type of decisions that I would want to find ambiguous direction about.  I would consider looking for another opinion about your course of action from someone who is more concerned about what is best for you and less concerned about liability.

  • I had an emergency cerclage at 16.5 weeks with my last pregnancy twins (we lost them 3 days after they were born at 24w1d-even with 8 weeks of strict bedrest).

    This time I have a transabdominal cerclage placed (pre-pregnancy, it's permanent).  My CL has been very long (as of 26 weeks, no funneling).  I'm 27.5 weeks now, and get checked every 2 weeks.  I am able to work, although I am part-time and spend most of my time resting when not working.

    I think you need to either seriously consider the cerclage and regardless of whether you get a cerclage you need to consider bedrest (strict bedrest).  You have already had a previous loss and even if they are not sure if that was due to incompetent cervix, you are already showing cervical changes so that needs to be treated.

  • It sounds like you need a new doctor who can better inform you of the options, risks and benefits.  A doctor who is more concerned about lawsuits than what is in the best interest of their patient is not a good doctor.

    It sounds like you have two choices though- a (semi?) risky cerclage, and bedrest.

    It seems like most of the women here are on at least modified bedrest even with a cerclage, so I guess it is really about deciding what you can most easily handle.  Can you deal with the financial repercussions of bedrest?  Or the risk of infection/loss (and ultimately still partial bedrest) with the cerclage?

    Sorry you are in such a difficult position!

  • I had my cerclage at 11 wk and 5 days. I had a m/c @ 17 wks with a singleton ruled incompetent cervix. THen saw perinatologist on monday at 12 wk 1 day and he said i was just prone to preterm labor. So my suggestion to  you is to do the cerclage. I don't regret it one bit and I know there are greater risks but sometimes we have to do what we have to do to get what we want in the end, which is a baby! GL with everything. I know its a hard decision...
