She's a little more than 6 weeks PP and started bleeding last night. She is breastfeeding so wasn't expecting her period yet (and didn't get it quickly with her first child either). She is worried that her internal stitches may have torn. Has this happened to anyone before? Is it unbearably painful if it happens?

It's most likely her period.
6 weeks PP she should be pretty much healed but I did have a friend who tore open her C-section scar at 5 weeks pp so you never know. My friend said it really wasn't painful. Your sister should call her OB just to be safe!

By 6 weeks she should be pretty healed. I actually stopped bleeding for 10 days and then bled for a few days around 6 weeks. I haven't had any bleeding since.
Definitely tell her to put a call in to her OB though!
Please tell your sister to call her OB today and let them know this. She should not be having bright red blood discharge at this point, especially if her lochia has been fading already and returned to red. Maybe it is PP period but to be on the safe side, it definitely warrants a call to the doc. Good luck!!!
Hmm. I'd think she'd have some other symptoms if her stitches have torn, and quite honestly, I'd expect her to be healed for the most part by now. My guess is it's her period. Was she doing a lot of strenuous activity or moving a lot yesterday? Sometimes that can cause a restarting of bleeding. Has it been a few weeks since she stopped her PP bleeding?
Tell her if she's not in a lot of pain to try and ride it out for a few days and see if it feels like a period. I can tell you this, the cramping I had during my first PP period after both of my c-sections was CRAP. Ugh.

I got AF at about 6 weeks with #1. I was BF'ing at the time. I was surprised when it came, but my OB confirmed it and told me that everyone is different.
I was looking forward to having a little more time to be AF free! I am not BF'ing this time, so I am sure it'll come early again.
I would suggest that your sister call her doctor because it could be AF, but it might not. If anything, it will give her piece of mind to talk to an expert.
Thanks ladies. She did call the OB this morning but the office is closed and she is not sure if she should page the on-call doc today or just wait until tomorrow.
She was sleeping when the bleeding started and has a little back pain and some pain near the incision but nothing major. Since she was sleeping she is not sure if the pain or bleeding came first. Also, her PP bleeding stopped about a week ago so she is thinking that it could be a little bit coming back and is going to give it another day or so to see what happens.
