in the hospital all they had were soft disposable cloths that you wet with water. at home we've always used pampers or huggies wipes. we don't use powder and so far no need for diaper rash cream.
I started off with Huggies wipes, but am now using Kirkland brand from Costco. I actually like them better...they are very soft. We use A&D with every diaper change for the same reasons pp mentioned.
I use the Pampers Swaddlers (I really like them) and yes we do powder DD's bottom (about every other diaper change). There is some controversy these days about baby powder, but we are very careful that she doesn't inhale the powder.

My hospital/pedi were very adament...nothing on baby's skin, even sensitive wipes, for at least the first month. We used Viva paper towels and water. After the first month we use Pampers Sensitive, agree that Huggies were a lot more wet/cold.

I'm a fan of Pampers Sensitive but a lot of my mom friends like Huggies. (I find Pampers to be thinner and less wet/cold)
We use A&D ointment after every change - it makes for easy poop cleanup and keeps the rash away.
