IC girls another question

Couple of question

1. For those that had IC w/ a previous pg, how did you kow something was wrong?

When my IC was discovered I waited so late that when I went to the ER I was in labor, but there was a lot of leakage. I didn't know if there was a way to catch it earlier?

2. For those that had IC w/ a previous pg when you were pg 2nd time how were you monitored?

I am almost 13 weeks and they are looking at me again at 16 weeks to see if I need a cerclage, I am wondering if that is a little late?

  • 16 weeks?  I had IC for a previous pregnancy loss and this time around my doctor said they will definitely be putting in a cerclage around 12-13 weeks. I thought the time frame was 12-14 weeks.  Did you doctor also say that you would have to go on bedrest with a cerclage.  Mine said that initially and now I'm praying that she'll say modified bedrest. 

     

  • They did my cervical measurements last thursday and they are going down but still "good" the dr. I am now going to is not "convinced" that I have IC, so she is sending me to a high risk specialist to make the call, but I am a little uneasy and looking at my origional dr. practice as they now have a high risk doctor there now. I feel like 16 weeks is so long from now, and this is nothing to play around with.

  • The best time for a preventative cerclage is 12-14 weeks.  If at 16 weeks things have started to change, then the cerclage has a lower chance of working.

    Is there anyway you can make an emergency appointment given that you are already 11 weeks.

    I had a transabdominal cerclage (permanent) placed at the end of August so my situation with my second pregnancy will hopefully be a little different.

  • My DD was born at 30.5 weeks after I spent four weeks in the hospital due to a bulging amniotic sac.  The docs never could say why I dilated, but they suspected IC/PTL.  

    With this pregnancy, my ob sent me to a peri at 11 weeks to decide what preventative measures should be taken.  My peri decided on the better safe than sorry approach.  So I had a cerclage placed at 13 weeks, started restricted activity/modified bed rest, and began progesterone shots at 17 weeks.  I'm 31 weeks now and  my peri says that there is no sign that this baby is coming early.  (Yipee).  I don't know why this pregnancy is going better, but I'm so thankful.

     Both my doctors and the general information on state that a cerclage has the best chance of being effective if placed between 12-14 weeks.    There is also more risk to the pregnancy the later they are performed.  My advice is to be proactive and don't wait until 16 weeks to determine if you need one. 

     As for symptoms, they are similar for IC and PTL.  Be on the look out for spotting, changes in mucus, braxton hicks/contractions, back ache, and/or a feeling of pelvic pressure.  I barely had any symptoms that something was wrong with my first pregnancy.  But I was just worried enough to call my doctor and go get checked out.  If I hadn't my daughter would have been born at 26 weeks instead of 30.  What seemed like paranoia at the time may very well have saved my little girl's life.   GL! 

  • I would really push for an appt sooner.  I had my cerclage placed with DD at 20w as a rescue and was told by my OB and Peri that I would have one at 12-13w with any future pregnancies. 

    I had no clue anything was wrong when I was pg with DD.  Because we knew there was a risk, I had them check the length at each appt.  At 20w, DH insisted they do it via trans-vaginal ultrasound after everything looked good abdominally, and I was down to less than 2 cm at that point.

With DS, I didn't know something was wrong until about 22-23 weeks, but only because my perinatologist (who I was just seeing for some genetic screening and to be released) noticed it on a routine ultrasound that my cervix was really short and funneling. My cervix was fine at 18 weeks. However, looking back, I remember feeling a lot of pressure, but thats about it. Ended up having an emergent cerclage at 23w3d.

     2. As soon as I became pg with #2, my OB referred my to the perinatologist who I ended up first seeing at 11-12 weeks. We scheduled the preventative cerclage at 14 weeks. Peri said it could be done between 12-14 weeks. I think I actually had it done at 14w2d, just because of scheduling. I believe 16 weeks is getting too late. Of course you can have a cerclage then but the further along you get, the higher the risks become. I wouldn't feel comfortable with that. Every since my cerclage at 14 weeks, I have seen my peri every 2 weeks for cervical checks and have OB visits 2-4 weeks apart depending on what is going on with my cervix, where she manually checks my cervix and my cerclage. So I am being closely monitored. good luck.

  • With my first baby I didn't know anything was wrong.  My perri happen to notice my cervix short and I had a cerclage done at 23 weeks.  For baby #2 he checked me at 12 weeks and I have to go back at 16 weeks for the cerclage.  He likes to do them at 15-19 weeks.  Good luck.
