I have 3 1 yr old bdays coming up and I am at a loss.
All girls and each one of the brats already has an AG doll with accessories.........so that is what I am dealing with. If that makes sense.

handmade is out as they already have everything I make and no time to order from someone else.

I'm trying hard to remember what the girls liked at 1 year. I can tell you it wasn't dolls though. It was several months into the 2nd year before either of them payed attention to those.
Let's see, at a year they are just starting to walk. Shoes? My girls loved the walk-behind / push toys.
Or do they have anything you could buy accessories for? Like boys with the Thomas the Train stuff? I say that because my girls couldn't care less about Thomas the Train.
What about PBK Anywhere chairs? I'm not a PBK mom, but DD1 loves hers and has to fight DD2 for it. They started in chairs around a year.

When she was 1, DD got a water table and a Fisher-Price busy ball popper and both were total hits.
Barnes & Noble has started selling books and matching toys, which I think makes for a cute gift. (Like the Llama Llama books plus a plush llama toy.)
If they've really got everything material, how about a zoo/kids museum/whatever membership and maybe a little toy reminiscent of the membership? Our family zoo pass was $45, I think.
I always do monogrammed Stephen Joseph backpacks for 1 year old birthdays. I HATE giving toys and really try not to. Actually, DS is even getting one because I don't want anymore toys in this house! OR, I give a box full of the felt clippies from either:
