What info do you leave for the babysitter?

We have a new sitter coming today and we haven't had a sitter in a LONG time (since moving to a new area).  I'm making a sheet for the fridge with my cell phone number, DH's cell phone, poison control and the pediatrician #.  What else to I need to leave for her?  I will be out running errands.  She lives in the neighborhood and her mom will likely be home too.

Not much.  We have a special 911 for our area, so I leave that.  I leave my neighbors number who will be home.  Her parents live 2 blocks away, so she knows a lot of people nearby.  She has both our cells and we have those near us all the time when we are out.

We have a little pre-printed "babysitter notes" pad.

It has our names, address (in case sitter needs to call 911 and give the address), phone number and cell phone numbers.  Pediatrician and emergency numbers.  Where we'll be and a number at that location (if realistic).  We also leave the number of our next door neighbor in case of minor emergencies (babysitter locked herself out once and the neighbors have a key).

Also has mealtimes/snacktimes and bedtime routine noted.

Everything the others posted plus special instructions on how to reduce a nursemaid's elbow.  For real, he gets a nursemaid's about once a week on average.  It's super simple to reduce it, but most people don't know how.
