With DC #1 I carried a MONSTER diaper bag every single place we went full of any thing we might need for any conceivable emergency. Even if I ran into the store for 1 item, it never occurred to me to leave the bag in the car--of course, it MUST be in my clutches every moment because what if something happened!?
With DC #2, I usually just keep the absolute bare minimum and I hardly ever take it out of the car--I figure if we are out somewhere, it really wouldn't be that hard just to run back to the car and grab what we need in the rare event we might actually have to.
I can't imagine what it would be like if we had a #3. At the rate I'm going, I probably won't even have a diaper bag. I'll probably just MacGuyver a few napkins into anything we might need along the way.

I'm the opposite: I actually carry more now with #2. I never even owned a diaper bag with #1, I'd just throw a few things in my purse or tote bag and call it good.
With dd though I find it easier to have an actual diaper bag and I use it quite a bit. With two kids I just have more stuff to carry around. Even though ds is long out of diapers I still need a place to carry his water bottle, his action figures, and all of the other stuff that he inevitably ends up toting along with us.
That being said, even though I do carry a diaper bag now, I never carry it around with me in stores, parks, etc. It generally just stays in the car.
No, mine haven't still the same size bag with all the same stuff if not more b/c now I have 3 in diapers. However my bag is very neat and tidy so it's really not a problem for me. I would say 50% of the time if I know it's and in and out job I'll keep it in the car for some stores so it's one less thing.
I have learned to put a luggage tag on it b/c I have left it in the cart before LOL!
