Just a few suggestions needed!!
1) serve coffee, tea, pop????-- not sure what to offer
2) couch or dining room table? my idea would be to sit in the living room as it is more comfortable and therefore less formal??

We sat at our kitchen table because it was easier for our SW to write her notes there. We asked her where she would prefer to sit and let her decide couch or table.
I say have the coffee pot ready to go (filled with water & beans or grounds) but not brewed yet. Have some hot water going, but no tea brewed. Ask the SW if he/she would like anything to drink and give the options and then either hit the brew button on the coffee pot or pour the hot water in a cup with a tea bag.

1) I would have all the options I could. Personally I don't drink tea or coffee but alot of people do. Our SW brought her own coke to drink.
2) We did our at the table because our SW brought a lap top and needed to plug it up and needed room to get her files out. I would offer both and let SW pick.

I offered coke, coffe, tea or iced tea and she took a coke. I didn't offer any snacks. I was going to and heard on here that she most likely wouldn't accept.
We sat in the living room on the couch. Much more comfortable.

I, too, would just ask her if there were something I could get her. My SW refused anything we offered, and even brought her own water, but I know many others would accept light food and beverages.
We, too, just asked the SW where she thought would be best to sit and talk. She chose the living room.
I put on the full court press! I got a snack tray at the deli and some fruit with cream cheese dip to nosh on. I had coffee, tea, soda, and iced tea available, but she just preferred water. She snacked some and we all had a good time chatting it up for 2 1/2 hours.
The kitchen table was best as our SW also had a laptop and notepad. Our kitchen table is also smaller than our dining room table and felt much more casual and comfortable. Had the weather been warmer, I might have even offered to sit our at our patio table.
Honestly, we had a really good time. We clicked with our SW immediately and, even though some of the topics were serious, we joked around and had a nice afternoon together.
