RE Appt. This Morning - So Confused - Help (Long)

I went in today for CD2/3 u/s and b/w.  I didn't discuss this month's plan with my RE last month, and in retrospect I guess I should have set up a consult (I just did 100 mg Clomid/Ovidrel/TI last cycle).  My RE's partner was filling in for him and my regular RE had not written down any plan.  On one hand it was good to have a second set of eyes, on another he gave me no clear direction and I'm super confused.  Here's what we discussed:

1.  I produced 3 good follies on 100 mg Clomid, but it thinned my lining.  We could do Femera, but I'd probably only develop 1 follie and Femera and this likely wouldn't increase my chances then since I'm unexplained.  We could do 50mg in hopes that this would thin lining less, or we could do Estrace later in the cycle with this (to be discussed next week with regular RE).

2.  DH's 1st SA was fine, but 2nd with Kruger was borderline.  1.5mL volume, and 4% morphology (but he said their lab is very strict, and 4% is the cut-off for normal). 

3.  My E2 has been a little high and he and my regular RE don't think it's b/c of DOR.  This RE said endo sometimes causes this, and if less aggressive measures don't work, we should do a lap before injectibles, etc.

4.  We should do PCOS panel b/c of my AMH being 6.3. 

As far as where to go from here, he left me choices of natural IUI since I O on my own, or Clomid 50mg/IUI.  He said I could do a lap in a couple months if that doesn't work before possibly moving on to injectibles.  He thought Clomid alone was pointless.  He said the choice was up to me. 

I guess I'll be doing Clomid 50mg/IUI if I can get DH to agree to IUI.  Otherwise, I'll do a natural cycle until I can convince him.  I just feel really confused and like I have no solid plan.  He pretty much told me that b/c we're unexplained all help up to injectibles or IVF will not up our chances much. 

Any insight?

TTC in 2008. Stage II/III endo, Hashimotos hypothyroid, low morph (3%).
2 cycles Clomid/Ovidrel/TI/Crinone=BFN.
IUI #1 - 4 Follistim/Ovidrel/IUI/Crinone = BFN.
IVF #1 - Antagonist w/ ICSI 4/10. 17 retrieved, 5DT of 2, BFN :(
IVF #2 - Long Lupron w/ ICSI 6/10. 15 retrieved, 3DT of 2, BFFN!!
Lap 7/21/10
IVF #3 - Clomid/Antagonist w/ ICSI 10/10. 14 retreived, 3DT of 3, BFP 10/20 but m/c. No HB 11/15/10 - D&C 11/17/10.
FET - 2 blasts, 1 survived the thaw. Transfer 2/19. Beta #1 3/1 375, Beta #2 3/3 885, Beta #3 3/8 4261, Beta #4 3/11 9005. U/S 3/8 1 sac 1 yolk, U/S 3/16 1 heartbeat 114bpm!

  • I'd be confused too. Maybe someone else has the answer but I dont see why a slightly high E2 would require a lap? Dont worry to much about the SA, it varies so much. If you O on clomid why not try adding estrace and doing an IUI? I dont understand why he didnt think injectables would be helpful, especially if clomid gives you lining issues. Its hard enough dealing with IF but when we have to deal with convincing our other half, well that just adds so much added stress. GL

  • i don't have any experience with PCOS or  high E2.  But I wouldn't worry about the borderline morph. It is very common. 

    good luck as you move forward. Take some time to do more research and think it over with your DH.  

    Ectopic pregnancy Aug. 2008, followed by a diagnosis of two blocked tubes.

  • I'm confused too!  You said the clomid thinned your lining, and mentioned using Femara, but then later said you might move on to clomid/IUI...?

    Just interested...why do you have to convince your DH to do an IUI?  If he's so apprehensive you might want to make sure he's on the same page with TTC before doing anything.  JMO

    The RE didn't think Femera would help b/c most people only produce one follie on it and since I'm unexplained the Clomid causing multiple follies supposedly is what would up my chances??  He thought that my lining would be thicker on 50 mg of Clomid than on 100 mg of Clomid. 

    DH wants a baby - he just wants to make sure we've "exhausted all measures" for trying at home before we move on to IUI.  We've only had 1 medicated cycle where we had TI.
