High-Risk Pregnancy

*Sticking my head in to ask a WWYD question...*

Hello ladies,

I've been back and forth between my OB, primary doc, and the hospital and my primary doc & the doc at the hospital thought I had the beginnings of pre-e. My OB insisted it wasn't pre-e, and thought it was a sinus infection.Primary doc gave me an u/s on my sinus cavities and showed me that it wasn't a sinus infection.

I've had some blurry vision, I am swollen in my feet, legs, arms, fingers, neck, and head (to the point I can't wear my regular glasses), I have had a headache that won't go away for days, and pains in my upper abdomen. When I get my bp checked its usually in the high 140s /low 90s but the OB had me lie down and it was 126/80. She pretty much said that the lying down bp was what she was basing treatment off of.

My issue is that everything I've been reading says that lying down on your left side helps to regulate the pressure and it's a way to get a false low reading- that all bp readings should be taken into account...

So far the OB has tested my urine and found "trace amounts" of protein but hasn't ordered anything else. My primary doc thought I should have a 24 hr test and buy a bp monitor for home. I'm thinking it wouldn't be a bad idea to monitor the bp at home myself, but does anyone else have any thoughts/ideas/etc?

I'm wondering if it's time to get a different OB- I'd hate to swtich but...

  • I would absolutely demand another opinion or followup testing.  There is no reason in the world your OB should dismiss these things without at least doing a 24 hour urine test.  You clearly know the symptoms of pre-e :), and have a right to be concerned based on what you've described.  Fingers crossed that everything is fine!  But, if I were you, I'd sure want to make extra-sure. . .

  • Buy a monitor and have your primary give you a 24 hour urine. It would not hurt anything and it would help make the decision of whether you needed a new ob or not. To me it sounds like you do, I had hypertension with dd and I never laid down to get bp.

  • Hey Heather! Fancy finding you over here. I'm sorry to hear you are still having trouble. I happened to pop my head in to ask a follow up question about my pre-e scare last week and I looked down a couple of posts and there you are. 

    I had to see the Nurse Practitioner last week when the protein showed up at a +1. That is when I originally posted over here. I went in for my follow up today and it wasn't as strong a +1 which is better then a +2 but not as good as gone. 

    I saw my OB today. Thus far my main symptoms are the protein and the headaches (especially when moving around). My OB told me that to get a totally clean sample they would have to catheter me. We aren't at that point yet. I am swelling a little but nothing crazy. I have upper abdominal pain but I had my Gall Bladder out 2 months before my BFP and that gets sore when I throw up which I still do nearly ever day. 

    Last time my BP was 118/72. That was a little high for me but not crazy. Today my BP was 144/60. That was just weird. I told them I was nervous about the appointment and had worked all day. They had me lie down and go to my happy place for a while. When they checked again my BP was 100/58. 

    If you look a couple of posts up at my post to Halo79RN I was asking her a question that relates to your PB question. It sounds like her issue at first was a bouncing BP. Check her response. It might be helpful! 

    Good luck with your decision. I am thinking more tests before axing the OB. If he won't do them then switch.

  • My goodness, I would DEFINITELY get another opinion or switch OBs.  For my last pregnancy I interviewed my OB and a  high-risk specialist about my issues and they had hugely different opinions, but I didn't personally agree with either of them.  I ended up seeing three different high-risk specialists during my pregnancy (none being the originally interviewed one) before I found a doctor I felt comfortable with.

  • As everyone else said, you need a second opinion. Sounds like you are experiences a lot of Labile blood pressure - that is when the BP fluctuates abruptly and repeatedly. This can happen before the BP goes up and stays up. It is tough when you are seeing 2 docs who do not agree. If you have to switch OBs at this point (keep in mind many will not take a previously seen pg patient without a referral.) try to see a high risk specialist who is familiar in dealing with pre-e patients.
