I normally just lurk, but something on your blog caught my attention.  I saw that D has reflux and is a noisy breather.  This is way out of left field for you probably, but ask her pedi about Tracheomalacia.  DD had a minor case of congential TM, and her main symptoms were the noisy breathing, always sounding/seeming congested, and reflux.  Her cries were also hoarse at times.  I trusted her 1st pedi that it was just normal newborn congestion, but then on a fluke she was diagnosed at 6w when she had a chest x-ray to rule out pneumonia.  They generally don't see it on xray, but she happened to take a breath at just the right time.  There's not much they do if it's minor, just a matter of finding the right reflux meds, and keeping her away from germs as much as possible, but it's something that is watched to make sure it's outgrown or if surgery is needed.  In our case, DD outgrew the deformity at 8 months, butis just starting to outgrow the reflux.
