DD does fine if someone can sit in the backseat with her. The only problem is she will not nap/sleep for longer than 30 minutes at a time in the car. Luckily the longest we've ever had to drive is 4.5 hours.
Just be prepared to stop every couple of hours to get out, change diapers, get a snack and just take a break from the boredom of the car. I find that as long as I don't feel like I'm in a hurry, it's not too bad.

We stopped a lot and let Lili run around. It still didn't help. We hardly ever drive, though, so that's more than likely what was wrong.

For DD the car seat is a sleep inducing drug. You put her in and she's out cold within 5 minutes ... 30 seconds if she's tired!
We also stopped every 3 hours so she could have a snack or eat.
