We got the Eddie Bauer breathable insert to keep him cool and in place. I think it was $15 from Target or something like that. We're still using it now, but he's starting to grow too much to use it.
We rolled up two receiving blankets to use along side his head until he hot a bit bigger, but he was a little less than 6 lbs when we brought him home from the NICU. He didn't "need" them, but they kept his head from flopping too much side to side.
lulabelle's head seems to be pretty well supported. that said; we went to the fire station and the firemen who checked our seat installation told us not to put anything that doesn't come in the seat because it isn't safety tested with anything that doesn't come with the seat and it could decrease the crash rating. we didn't want to risk it.
the seat is great though. i love that it just clicks on in!

This, although DS was 5 lbs. 14 oz. when we left the hospital. We still have the headrest in the seat and he is 17.5 lbs.
