I believe Tom Thumb gives you one if you order the larger cake from them.

Albertson's will give you a free little cake if you order a 1st bday cake from them.  They also have pretty yummy cakes and can make pretty much whatever you want. We've gotten the Baby Einstein caterpillar (circles put together), a Mickey Mouse face shaped (3D look), and a SpiderMan face.  They've all been really good.
