Take your # of stitches per inch and multiply it by the number of inches you want and cast on that number. Remember the stated gauge may be expressed in the # of stitches over 4 inches. Then make a fraction of your cast on number over the called-for cast on number (say 1.25) and multiply the numbers throughout the pattern by that number, fudging here and there to get whole numbers.
Alternately, you might be able to knit with the same numbers using a larger gauge. Just check and make sure that the gauge you want gives you the size you want.

Sometimes the pattern will tell you the pattern is in multiples of X stitches.
Or you should have a gauge measurement, so if you're going up a certain # of inches you increase by the # of stitches to get an inch in a gauge.
