I've been dealing with the same epiphany as you although my two girls are a little bit older than yours. My oldest will just have turned 3 when the twins are born. I'm both excited and terrified. I'm glad to know there is someone else facing taking care of all these little kids at once.
It can be done! My oldest turned 3 a few days before my twins were born and my younger DD was 19 months. It's actually better having two older ones that can entertain each other while you're breastfeeding, rocking babies to sleep, etc. I'm so thankful they have each other! Overall, it's not as scary as I thought it would be, and I've managed to get out of the house a lot on my own. Good luck!! Every day is an adventure. Oh, and my aboslute salvation is the fact that both older DD's take a good 2 hour nap every day together, and go down without a fight. Let me know if you have any questions or need advice on anything else! I'm happy to know I'm not alone, too!

Oh, and I wanted to add, as far as your other concerns:
Two births at once was no big deal for a third delivery - for me, at least. My first two were vaginal, and so were the twins - very fast and easy. As for the 4 carseats, we had to buy a minivan. We have a Dodge Caravan with the older girls in the 3rd row and the infant carseats in the middle row, and it works. Not all carseats are LATCHed though. Not a big deal, in my opinion. Next up, buying a bigger house

Thank you so much! We have a Durango with three rows, and I can but the two older girls in the third row (not latched, but we'll make due) and the babies in the middle. Did you have to buy a triple stroller? Thats something I've been thinking about (for some reason.....there are so many more important things!!!)
