Sleep help please!!!

I don't know how to get her to sleep on her own.  I try to put her in her basinet or packnplay, and she will not sleep long in there.  If she sleeps on my chest, she will sleep for 4-5 hours.  Usually, she will sleep on my chest until 2-3 am, then we move in my bed, and she is up every 2 hours.  But I BF, so basically I just pull out the boob and go back to sleep.  Last night I put her in her packnplay, and she slept for 1.5 hours.  Then was up and ready to eat, and didn't want to go back down on her own.  So she slept on me for 5 hours.  Any advice? 

DH is home with her during the day... and he is working on getting her to nap in her basinet or packnplay.

we are in the process of getting DD to sleep in her PNP rather than the swing (where she will STTN), we have tried several times and she would wake up after 20-30 min. The last 2 nights she has made it about 5hours, so I think she may have just been ready because we didn't do anything different.

We give her a bath, swaddle, bottle and then to bed drowsy but awake.
