So, how did Meredith know that John Doe was George?  What does 007 mean?  DH and I have been watching since the beginning and neither one of us could figure that out.

Also, the end scene...does that mean that both George and Izzy died?

I was just getting to like the Alex/Izzy relationship.  And now they're going to go and bump her off!

That's his nickname from early on when I think one of his patients died.   Alex was referring to him as 007 last night too.

I don't think they both died.  Maybe George, but I still think that Izzy is coming back.  Yes, she wants off the show, but she's a popular actress and is still contracted, so I think they'll keep her.

I just read an online synopsis, I didn't finisht he whole show.  I also read skorb''s info and she says george went by 007 when he had a patient die - I don't remember that either....

I'm so bad to read the spoilers, I'll have to watch the last half this weekend.

Since both Katherine Heigl and TR Knight have been so vocal about wanting out of their contracts, the show was written with a cliffhanger about whether they'd be back.  Apparently the creator is so pissed off at them that she hasn't let either one know whether they survive and has until the end of June (not sure why -  maybe it's when they start shooting again?) before she has to tell them.  If you believe the rumors floating around, George will die and Izzy will live but who knows.  Personally, Katehrine Heigl iritates me so I hope she's gone and TR Knight hasn't had a storyline on the show in forever so I really won't miss him either.

and the 007 thing... Season 1, George killed at least 1 patient, maybe more and I think it was Alex or maybe Christina who called him 007, saying he had license to kill..

The 007 was a reference to one of the first episodes, where George got to perform an appendectomy alone, and all of the other residents were watching. ?He almost killed the patient, and was called 007 - "license to kill." ?I was also liking Alex and Izzy - no idea who is going to die!
